L-carnitine, also known as L-carnitine, L-carnitine, or vitamin BT, is commonly referred to as levocarnitine in clinical settings. Its chemical name is β-hydroxy-γ-trimethylammonium butyrate. It is a vitamin-like substance widely found in nature and was first discovered in muscle extracts by Russian scientists Gulewitsch and Krimberg in 1905. Carnitine exists in two optical isomers: L-carnitine and D-carnitine. Only L-carnitine has physiological activity and is naturally occurring, while D-carnitine is synthetically produced and does not exist in nature. D-carnitine not only lacks biological activity but also competitively inhibits L-carnitine. Since the 1990s, the global market has not approved the commercial use of D-carnitine. Therefore, the term “carnitine” usually refers to L-carnitine.
